DEPARTMENT OF THE INTERIOR
UNITED STATES GEOLOGICAL SURVEY
WASHINGTON

OFFICE OF THE DIRECTOR

Hull House Association,
Polk and [Halsted] St.,
Chicago, Ill.

Gentlemen:

In cooperation with the United States Fuel Administration the Geological Survey is undertaking to obtain information concerning the operation of private electrical plants. To this end I am sending you a schedule of inquiries upon this subject with the request that it be properly filled out and returned at your earliest convenience in the [enclosed] envelope, which requires no postage.

The schedule is being sent to all plants generating electrical energy which are not operating primarily as public utilities or are not recognized as such by public authority. In order to meet the great variety of conditions found in private plants and at the same time obtain the information desired it has been necessary to include a fairly large number of questions. It is entirely possible that some of the questions asked may not be applicable to your plant. In such cases, please indicate this fact in the appropriate spaces.

It is also possible that this request may reach some concerns which do not generate electrical energy. In that event, please return the schedule with a statement to that effect.

If the schedule does not give proper opportunity for describing adequately your plant and equipment and explaining the manner in which it is operated, I will welcome a supplemental statement on the subject with any further details which you may find it desirable to include. The schedules are sent to you in duplicate in order that one copy may be retained by you for reference.

Very truly yours,

Geo. Otis Smith [signed]
Director.
