Dear Miss Addams:
Unfortunately I did not get in touch with you while you were in New York to ask your permission to include your name in the Dinner Committee issuing the invitation to the Survey Dinner on April 27. Our request instead went to your Chicago address. Since there was not time to wait for a reply from you, we took it for granted that you would have no objection to so using your name and trust this will meet with your approval. The invitation to the dinner is undoubtedly in your hands by this time.
